8. CHOLINERGIC URTICARIA

A common condition in young adults with intensely itchy and short-lived eruption developing in response to sweating, exercise, emotion and hot foods. It is postulated that an increase in blood temperature triggers a neural reflex which releases acetylcholine from sympathetic nerve endings, in turn activate the mast cell to degranulate. Characteristic small wheals, less than 2 mm in diameter with surrounding red halo are more profuse on the upper trunk and proximal parts of the upper limbs. Thus it is also called micropapular urticaria. Associated systemic symptoms include faintness, headache, wheezing and palpitation.

Diagnosis is established from the history and the finding of characteristic rash during an attack. The rash can also be brought up on exercise or whole body warming. These lesions can often be reproduced by intradermal injection of cholinergic drugs, e.g., metholyl or acetylcholine.

Treatment is unsatisfactory. Patients, especially those with associated systemic symptoms, should be told to avoid situations that can precipitate an attack. Some patients improve with antihistamine therapy. This can be taken regularly or at times when they anticipate attacks. Fortunately for most patients the condition tends to improve spontaneously.

It really sucks.

I too suffer from cholinergic urticaria. I have tried the antihistamine Hydroxyzine, and it helps very little. If I have any kind of emotional stress, good or bad, I erupt in hives all over my chest. It is quite embarrasing. I don’t know what to do anymore.

What ended up working for me was fish oil pills (the omega 3 fatty acids EPA and DHA.. not the plant forms, they aren’t the same thing). I’ve gone weeks without taking them, it but it seems like if I go too long, it starts to come back(only a fraction of what I started with but enough to scare me into starting again without seeing what would happen if I went longer). Within about two weeks of consistently taking two or three pills a day (I just took two each morning) you should know if this is going to work for you..if not, I’d move onto something else. Theoretically, it would also help to reduce omega 6 fatty acid consumption at the same time (the omega 3/6 ratio is important) and of course, try to avoid overly excessive stress/caffeine. Sweating occasionally is generally good idea too, but I have no idea if the histamine dump and 1-day refractory period influence how long it takes for the pills to take effect.. I don’t remember making any particular effort to work out or get hot when I started…but I suppose it couldn’t hurt.

hi i have the same problem with blushing and a really red blotchy neck that lasts for ages after ive stopped blushing i have been using eradicane for the past fortnight and i think its working i have just found some other tablets from the same company called hivecten they are supposed to stop the neck and chest area going red and blotchy i think im gonna try it and i will let you know what happenes

i have this problem too..however i have tried out couple of things that have made this problem bearable to quite an extent
- Get some homeopathic treatment ( http://www.urticaria.com). Ive tried them..i get ok for a month or two then the problem started again. again. However i had a relapse and then the medicine made me better but not OK. Have to go to my doctor again. Will try a different medicine. Homepathy is the ONLY cure for cholinergic urticaria.
- Psyillum husk ( fiber suppliments) - can make ur condition better by 50%
- Exercise (Sweat it out) - Will keep u going for the next 20 hours without problems.

I have chronic urticaria. It presented over a year ago. I am 55 years old. I am taking antihistamines but they are not working. I love to walk but the hives are driving me crazy. If anyone has suggestions on what to do please e-mail. I am also concerned about taking antihitamines for a long period of time.

Hi, I’m 32 and have been a keen runner for many years. That is until recently!. Everytime I go running, I get this rash and last around 24-48hrs depending on how long I ran for and therefor my body temp would have been hotter for longer.

I have no diagnosis yet of what it is but having researched the subject on the net, This condition sounds very similar to what I experience.

I am also following a special diet but it doesn’t appear to help.

I hope it goes away - and soon! I know how you all feel. Its no fun whatsoever is it..

Antihestamines just don’t appear to help. I do use “Eurax” though which you can buy over the counter from most chemists. It has helped my itching, though I am getting through lots of it!.

Talking of which, I’m off to apply my second lot of the day.

Hey there, Ive had cholinergic urticatia for nearly a year now also. I’m currently seening a very good Naturopath (homeopath) and it has really helped a lot. I’ve come to realise that this condition is very much related to my sleep, food I eat, and general stress. I used to have very bad sleeping habits, and also live a stressfull lifestyle. Interstingly, I was cured about 6 months ago, it just spontaneously regressed, but i started sleeping badly again, and eating lots of milk and cheese (I am allergic to milk), and the condition came back again. I think the best thing is get into a routine, dont stress about it, and try and eat and live healthy, and see a good homeopath.

Hi, I have been diagnosed with cholinergic urticatia and have now suffered for 18 months. There has to be something that can stop this! I’m going mad! I take antihistamines everyday and it is proving very expensive. My doctor just seems to think that this will go away but so far it hasn’t. Please if anyone has any ideas of what could help, get in touch… I’ll be your grateful servant forever!

Hey again, I’ve finally found a cure! I started seening an acupunctuist 7 weeks ago, and the urticaria progressively became better and better. It is pretty much completely gone, and I only have 2 more appointments with her. Seriously, acupuncture is the cure. Just make sure you see a good acupuncturist, who is well established and has a good reputation. I am seeing Emily Teo in Sydney Australia. If anyone has any questions, just make a post, and I’ll hopefully reply soon.

I’m 16 and Ive had this thing for 2 years now. Its not so much a health problem, its more of an annoyance. I find it to be annoying and especially when u start scratching ur head and everyone thinks u have lice. Hot showers each morning seem to help, but usually i can work it all out in about a half hour each morning and be fine for the rest of the day. I find it to never bother me at night time, If it does its very rare. Hopefully its cured fully someday, until then we all just have bad luck for getting it.

I have this condition too. Very annoying. I’m 29 and have had this condition since high school. It manifests itself most when I exercise. I went to an allergist a few years ago and he prescribed Claritin (loratidine). It takes 5 to 6 days to get into your systems, but I’ve found it helps. When I exercise, wearing tight spandex clothing helps b/c the clothes don’t rub on my skin to further irritate. Also, when I’ve been under extreme stress, I’ll have an attack. Someone can just touch my arm and it goes up in hives.

I just started having hives when I start exercising. I am 27 and have worked out my entire life. What kind of doctor did you go to? I haven’t been able to figure out how or why this started. Any advice would be great.

I have had urticaria since mid July 2004, Just today I went to see my 6th doctor(dermatologist) and he said I have what is called Cholinergic Urticaria - he said don’t do anything that can cause sweating. I have taken lots of varieties of antihistamins - can anyone really help regarding this CU ? Any cure at all ? please email. Thanks

I just found out i had the big CU yesterday! Before then all I knew was when I get hot or stressed I break out in a painful rash. The doctor wasnt a big help but il be starting some meds soon, well see how they help but im not holding my breath. From all the web-sites ive looked at and people I talked to the general thought is ” this really sucks and theres not much you can do about it!” Well anyone else out there that wants to join me in my pain can e-mail at Fiersw@yahoo.com….. Hi, all, i had this Heat induced Hives problem for over 12 years now, i tried different diets, antihistimines but nothing seems to get rid of it, But recently i visited china and gotten prescribed with 2 medications:Desoloratadine(Clarinex)-H1 receptor and ratadine H2 receptor that actually Stops the Hives from coming out. The doctor said to exercise and sweat it out after taking medications, But i’m quite a lazy person thus i could keep a consistant exercises schedule, so the Hives still breaks out if i don’t continue taking the medication. On the bright side i now have a medication i can rely on and i was able to dance 2 hours continous at night while sweating without hives breaking out. With the little exercise of 1 hour a week i put in , my condition has improve alot, i would say 35% less red dots/hives and 70% of all red dots/hives are located on torsoes/arms/legs area. I’m 34 and living in India and i too have this chronic urticaria for the past 8 months. u could see small eruptions in the head scalp and lot of dandruff formation plus i get itching all over my body which turn into rashes. I dont know which started this Urticaria in me. May be the swimming pools or the dust thro which i drive my bike to office or the silver filling that i have i my teeth or the excess Airconditioner i have in my office(Driving my bike in dusty roads and airconditioner r new to me coz i started working jus 8 months back). Eventhough i get rashes all over it mainly comes in the left side of my body. The left arm, the left side of the neck. Is it due to a small lump in the left side bottom of my head? coz sometimes i get bumps as how u get when an insect bites in my left hand. Now i use NYLE shampoo with conditioner and the dandruff and the small small protrusions in my head is better now. Antihistamines helped me but after stopping them the rashes appear again. I visited a homeopath and he gave me medicine thinking that it could be due to swimming in swimming pools some 8 months back. Now it is better for me, but still i’m not completely cured. Shower to shower prickly heat powder also helps me to a great extent. But one thing i know for sure. One night after a bath, i had rashes all over my chest and back. i looked myself in the mirro and i cried to Jesus, Lord, why this pain and rashes for me. i Cried bitterly and told him to forgive all my sins. From that day, i dont get rashes and pain in the night. He gives me sound sleep. Before i spent many sleepless nights. I’m sure he’ll soon wash me off from this Urticaria. “My Jesus I Love U”

I have had this horrible thing for over 5 years now. I lose alot of sleep and I am very itchy all the time.I have thought many times of just ending it all because I itch so much and I can’t sleep. My husband is suffering right along with me. And I break out in these little bumps that get red and itch like crazy. I am alergic to almost everything so medicine isn’t going to help me. I am going to the doctor again in a few days and I just know that he isn’t going to be able to help me. I don’t feel good most of the time because I am so tired. I am angry that this has attacked me and will not go away. I can’t pin point what is causing this. It isn’t heat, it isn’t cold , it isn’t sweat and it isn’t exercise. I know that stress plays a part but I get them even when things are great too. Help me please!!!!

When it hits, it is more than I can tolerate. Later, I go on with life and hope it doesn’t happen again. Then it comes back. I go from Dr. to Dr., trying to find another diagnosis. I’m tired of looking. Why me? I love exercising and activities in the sun? Is it better to die in the light?

Try to take something called Yeast Away. It works for chronic cholenergic urticaria. I have had them
for 10 years and have had all of the antihistimines, prednisone, h1, h2 all the medications you can think of.

I think from what I’ve read that I have some sort of urticaria condition. I break out in hives on
my neck, back, and chest after showering (warm water) or after sweating. For me, the hives don’t seem to be related to stress or diet. They last anywhere from 15 minutes to many hours. They worsen if I
scratch. I am looking for something to do to prevent them, rather than treat the hives after the fact. Can anyone help me?

Hi,i get itchy everytime i exercise or get startled,it comes on after about 8 minutes if i exercise.I get this prickly,itchy sensation on my head,back and shoulders.If i drink tea or coffee,i am asking for trouble.If i walk into a hot room i get the itch!.Sometimes i can get it if i walk in the sun.It comes on then disappears after a few minutes, even if someone just scares me! The difference between my itch and all of yours is that i have no visible rash, there is no hives or marks. Hi, another one on the list! ive had it since 2001. funny story too. I was trying out for volleyball and on the last day of tryouts i worked my butt off to make up for the mistakes i made for the rest of the week. then i blew up like a balloon, i was one big hive. I didnt make the team either. since then it has gotten increasingly worse every summer. and bummer too cuz i live in texas. Its always at or close to 100*. but i have had problems all year round. its a change in temperate for me, and when i exercise. even getting in the pool will freak me out. i had to stop exercising all together because nothing will work. ive been on every kind of antihistamine known o man and NOTTA thing worked. i cant even go outside now cuz its 100 (well 105 with the heat index) I cant live my life like this anymore!! any suggestions??? i need to work out again but the hives are overwhelming. email me… elisah13@hotmail.com

p.s. my symptoms are brought out by , embarrsment “which the utricaria more than usually causes) anxiousness, general stress, i have come to believe that my thoughts have an over whelming say in how my urticaria works, i believe that if i control my thoughts i can control urticaria, (easier said than done) i may even go and see a hipnotheropost, i am determind to beat this problem and one day i will

First, thank you for all the postings. I have had this for several years and I think it is linked to 3 things:
(1) Stress - excessive stress leads to itching
(2) Diet - dairy ans sugar cause me to itch
(3) Sleep - poor sleeping habits
When anyone of these three things is off in my life the
itching and the rash begins.
I am not cured but my problem has decreased significantly.

i have urticaria (hives)and im 16 yrs old now.I had this when i was about 5/6 in the end it went away, last year when i was in year 10 i started gettin it back really bad, i went to the doctors they said i had hives. They gave me pirton in the end they stopped working.then in the end they went away becase they gave me zertect, they made it go away in the day but when i when to bed it aways came back! in the end i found out i was allergy to fizzy drinks, so of couse i stopped drinking them. A few day ago i hav got it back, i have not drank anything i shound not, im starting to get dressed again i cant go out any where i feel really silly. Its my friends birtday on friday i dont think i can go to that any more, i cant where any thing which i want 2 where becuse i hav 2 cover up my skin. im takeing all different thing at the min and none of them are workin i dont no what to do?!?, i stared having dounts that i am not got an allergy to fizzy drinks, can u hav an allergy to 2 things? (petits filous are the only new thing i hav been eatin, but they are all natral) could this be whats up? how can you make hives go alway? please i need someone to help me! i cant stand it any longer!!! plz EMAIL me yours faithfully michelle

my daughter is 16 years old and 2 days back she had this attack of acute urticaria she has been prescribed ketasme and sizon forte and has to follow many food restrictions . this i feel is very difficult for a teenager as the medicine makes her drowsy. pls tell me how to avoid it to become a lifelong problem .

I am 25 and my CU started a year and half ago, It started with one big hive after running. Then it started to show everytime I exercise and it wasnt only one but a lot of them throughout my back and chest then in my arms and is starting to show in my legs. I have tried a couple of anthistaminics and didn’t work. I get this CU when I have a stressfull situation, and mostly is related with body temperature. Every time I do exercise the hives will show and the itching. If I excercise in the beach I don’t get my entire arm with hives, because there is cool air blowing to my body, but if I run or play a sport with a swetshirt or in the sun, my arms look like a galaxy, full of big hives all over my arms, chest, back and some in the legs. I have long hair, so I don’t know if they show in my head but I do get the itching in the head. After 30 to 60 minutes it all goes away without a trace and the itching will go away as well.If I do indoor sports the attack is very severe and it takes about 80 minutes to go away, but after they go away it seems that nothing happened. It is emmbarrasing because I cant play that much indoor sports because the temperature in the room is mild or warmer than outside and that triggers my hives to a severe level. I have had UC for 8 years and the hives occur on my muscles on my lungs and throat. So i do get red flushes at time but mainly it hurts to breath and too much movement (such as walking to the shops) is painful and very very tiring. When my CU is triggered by things like cig smoke, MSG, food dyes, perfumes etc i take a sedative antihistamine and a montelukast tablet (often given to asthmatics - it helps controls mast cells)at night and a slow releasing antihistamine each morning and try not to exert my self at all until I feel better (one day to a few weeks later). Definetly need a strong mind to help get thru this illness, so keep a stiff upper lip and listen to your body and learn your triggers and keep searching for those little things that help. Hi to all,
What a good website. I had hives approximatley 17 years ago, and they just disappeared after 2 years thank goodness, we went through all the usual washing powder, what you eat,temp, and nothing was found to cause this infliction. But they started again and I had forgot how binding (for want of a better word)it is.
I know it will go away, but it takes time and feels like a lifetime, so I hope for me and you this will happen soon.
Antihistamines do help a lot of people to make this bearable, I am pretty sure that stress has a lot to do with this also, which does not help because it is a stressful condition anyway.. ( we are going to emigrate soon from England which is a big change)
I am usually lucky that most of my clothes can cover up the condition, but today was a bad day in which my lips swelled and I had to go to the hospital, they changed my medication, I only hope the condition subsides enough to go back to work.
sometimes they can dissappear for hours, and you live in hope that maybe this time they are gone!
Well I send all my best to all of you, and hope you email me to tell me how your getting on, and if I find anything new to eliviate it I will let you know, but remember although it is exasperating, it can go away and you will get pbetter..

Hi I am 25 years old and have had CU for about 11 years now. Pretty much i get the symptoms in response to a variety of trigger that increase my body temperature. This includes warm weather,hot showers, spicy food,also some fruits and juices, exercise, nervousness, stress etc.. i am hoping i will grow out of it as people say but i dont know. I am currently taking allegra daily and atarax occassionally. these only seem to ease the symptoms but are far from curing it. i am a student so it is also difficult for me to take the atarax daily because it makes me extremely drowsy. i would do anything to find something to make this CU go away. obviously the allopathic approach has not been has effective as desired so i am interested in learning more about the homeopathic approaches. please share anything that has worked you any of you that read this post. my email address is sheripatel@hotmail.com. thanks

Hey everyone, I have had CU for over a year now and while that is relatively not that long to have the condition, I have gone through a lot of different approaches to cure this problem. Like many of you, I began with the allopathic approach which helped very little. One thing that has proven fairly effective, however, is Periactin, a highly powerful H2-receptor that causes drowsiness. Recently, I have started the homeopathic approach and am still awaiting its highly praised results. Urticaria - I hate that word. I had been diagnotised by clever nurse (NOT MY DOCTOR) during high school 8 yrs ago. I was very active during those days. Mostly it was just an annoyence. But one day during a long and hard KATA in a Dojo exercising to my limits I almost passed out. I couldn’t even breath and I can just imagin how my face looked like (stung by millions of bees). Since then I just stopped exercising. It helped, since there can be long periods before I even become reminded that I can get rashes in an instant if I just sweat.

Also during this long period I found that there are things that can be done to ease the problem. I liked specially acupuncture/acupressure and meditation. They are definetly no cures, but as long as you “think” they “are” it will help. Rashes might dissapair, if they don’t you’ll not become as irritated/annoyed. Specially acupressure and medition is very cheap solutions, just google “urticaria and acupressure”. The sameway it goes with homeopathy, except its a very expensive solution. Hell.. If you think that sugar in water helps, I suggest you take with you a bottle everytime during excersise.

I am a registered nurse and student homeopath and I saw this website when looking up information about cholinergic urticaria for my son. Both acupuncture and homeopathy can help this condition, but you need to see a qualified practitioner because the treatment is holistic. Antihistamines and corticosteroids remove symptoms but do not treat the cause, so the symptoms come back.

Im 25 years old and I too have cholinergic urticaria. I have suffered from this condition for years and years now - its driving me nuts! The hives appear from my waistline upwards everytime I have have a shower, go swimming and go to the gym. Its very embarrassing in the gym when I come out in red slotchy hives and start the itching can be intense. I believe I have a an allergy to my own sweat as I also break out in an acne type rash on my forhead. I recently started to take Clarytin and its seems to be doing the trick. My forehead is nearly compltely cleared up and my hives are not appearing after a shower. I have yet to go swimming or to the gym, so that will be the real test. Good Luck to those of you who have this and hope a cure is found soon.

Hello everyone, i feel for each and every one of you out there sufffering from this CU. I have read each and every single post, and i was just wandering if by any chance this condition has interfered with anybody’s ability to sweat. The reason that i say that is that whenever im anxious, become warm, or whenever my body temperature rises, i get this pricking, and unless i get my heartbeat down to normal again, the tingles will just come out stronger and stronger and than ill feel that intense burn and itch, and the worst part is that i become short of breath. If i push it to much, i feel like im gonna faint and my throat feels constricted.

I have had CU for approx 3 years now and it tends to be worse in the fall and winter for me. i have tried numerous antihistamines and corticosteroids. I have seen 2 allergists and they are unable to help me. I love to workout and enjoy the outdoors. I have recently tried quercetin which is recommened by many herbalists. I have had no luck with this yet. I too break out in hives when my body temp rises no matter what the cause. I mainly have them on my arms, legs, back and chest. I mysteriously was hives and itch free approx 5 months ago. I do not know what I changed, but the hives came back 1 month ago. I appreciate any other advice on this annoying problem.

I discussed CU with a local herbalist who recommended using MODUCARE 3x daily. He almost swears this will solve my problem. It is an immune system balancer. It does not suppress the immunce system like many Rx drugs out there. It will not make a person more susceptible to infections or viruses. He mentioned to give it 1-3 months to see the full effect. It is available at a local health food store or it is much cheaper online. Just go to google and type in MODCUCARe. It is all natural and is used for many auto-immune problems.

Yup I was diagnosed with CU about 2 years ago. I found that taking cold showers helps a little bit, if you haven’t tried that it may make things a litte more bearable.

Yeah, I feel with you guys. I get this itching after warming up or any kind of sport mainly on my chest and back only for 2 minutes or so. Also in swimming pool after swimming craul, which was for me interesting, because the water was cold. I had that also after skiing, when I moved a lot, so I had to stop release my clothes. I can have itching in a minute when I want, just by moving fast. I love to exercise, but this itching demotivates me. I have this for 2 months. Is this illness infectious?

I recently began having these hives on my back, neck, arms and chest. Ocassionally they appear on my legs and worse yet, the bottoms of my feet!!! My problem isn’t itching as much as pain. Even after the hive has disappeared I can feel soreness at the site it appeared, especially if it was a larger area. I have been prescribed Atarax but it makes me groggy the next day. I’m going to try Claritin daily as suggested on this site.

My question is, does anyone find these are painful? Almost to the point where where it appears (especially on the neck) to be affecting deeper tissues or possibly muscles?

Feel free to email me, I am at my wits end and need to know if anyone has the pain as well.

As i said 3 very long hard years and i would have given anything to get rid of them. so i have to let you know what happened for me.
I had Shiatsu. with almost no physical contact it took my body to a place of well-being, where it felt easy, a place it knew a place it felt at one. I’m not a writer so its hard to explain. IT WORKED FOR ME.

I had an outbreak over the weekend. It starts out with my head itching like having million of mosquito bites, then it goes to my neck, hands and then it is overbearing. It sometimes burns and hurts. I have had breakouts before but this time my tongue swelled up as well. All I did was go for a walk. Thank God I had benedryl which saved me. I did go to the ER because i got light headed and faint. This is a horrible thing to have and no one knows what causes it.

I keep searching sites to try to see what others are doing since the doctors are not much help. Zyrtec is keeping me alive. I just burn everywhere, not alot of itching and am red skinned, people think I have a sunburn. I wonder too if it will stop.

After hot showers, I get intense itching on my legs with red blotchy patches. Lasts for an hour or two. Seems like CU. I apply baby oil and that seems to help a bit to reduce the itching. I am going to try cooler showers and maybe stop eating suger and dairy.

Hi.Before today I thought I was alone and that no one understood what I was going through.I was recently discharged from the Medical College of Georgia where I was having a study done on me.I have been suffering with this problem for going on 8 years and no one seemed to know what was going on.I was sent to every kind of doctor (Endocrinologist,Allergist,Neurologist,etc.), thinking it was seizure-related.I have not been diagnosed yet, but I believe, after researching, that I have found the answer.
Since I was in the 6th grade, at the age of 11, I’ve been suffering with the itching, tingling, hives, headaches, etc.When I passed out the very first time no one believed me about seeing “yellow people” (an aura) so I was sent back to class and eventually ended up in the ICU for three days.
This has prevented me from having a normal life.I ws ostracized in school, unable to do anything physical, such as cheerleading, PE, or softball.I feel like I am completely out of options and am seeking advice.Please feel free to e-mail me.

Like many of you, I have been suffering from cholinergic urticaria for approximately four years. My doctor suggested that stress my be contributing to the relapse of my symptoms so I am writting a summary of my experience in an attempt to vent my frustration and anger…hopefully taking some stress off my mind. I’ve tried every possible treatment like corticosteriods, antihistamines, and herbal medication too. What angers me is the herbalist gave me false hope by telling me he could cure me (profitering from my pain!!!). Other than this I have had no success. I have been fortunate to be symptom free (pain and hives on occasions) for about 8 months, however, recently since the start of the winter period my symptoms have returned. The symptom that bothers me the most is the prickling pain that I experience basically all over my body. I have been using Anti-inflammatory medication which have reduced the pain slightly (speak to your doctor before considering this option). But mostly, I’ve been running as hard as I can in an attempt to help my body develop tolerance to “sweat” and to prevent my body from SUPPRESSING sweating (this happened at start of my condition). I used to play sports (before I was struck down with the condition) because it gave me a sense of freedom - no boundaries.
